1. Acceptance of Terms
By accessing or using the Quantum Polyclinic website and online booking portal, you agree to be bound by these Terms of Service. If you do not agree, please do not use our services.
These terms govern your use of our website at quantumpolyclinic.ie and the appointment booking services provided through healthcarebookings.com on our behalf.
2. Our Services
Quantum Polyclinic provides private diagnostic imaging and clinical services, including but not limited to MRI, CT, ultrasound, and X-ray examinations. Our online portal allows patients to search for available appointment slots, book appointments, upload referral documentation, and manage their bookings.
Booking through our portal does not constitute a medical consultation or diagnosis. All clinical decisions remain the responsibility of qualified medical professionals.
3. Eligibility
You must be at least 16 years of age to create an account and book appointments independently. Bookings for patients under 16 must be made by a parent or legal guardian, who accepts these terms on the patient's behalf.
4. Appointments and Bookings
Referral requirements: Certain diagnostic services require a valid GP or consultant referral letter. It is your responsibility to ensure you have an appropriate referral before attending your appointment. We reserve the right to decline to perform a procedure where no valid referral has been provided.
Accuracy of information: You are responsible for providing accurate personal, medical, and contact information when booking. We are not liable for any consequences arising from incorrect information you have provided.
Confirmation: Your booking is confirmed when you receive a confirmation email and booking reference. Please retain this for your records.
Attendance: Please arrive at least 10 minutes before your scheduled appointment time. If you arrive more than 15 minutes late, we may be unable to accommodate you and your appointment may be forfeited without refund.
5. Cancellation and Rescheduling
Appointments may be cancelled or rescheduled through your account on healthcarebookings.com or by contacting us directly on +353 (0)91 379 200.
Cancellations made with less than 24 hours' notice may be subject to a cancellation fee. Please refer to our current fee schedule on our website or enquire at reception.
We reserve the right to cancel or reschedule appointments in exceptional circumstances, including equipment failure, staff illness, or events beyond our control. We will notify you as soon as possible and offer an alternative appointment.
6. Payments
Payment is due at the time of booking or at the time of your appointment, as indicated during the booking process. All prices are displayed inclusive of VAT where applicable.
We accept the payment methods listed on our booking portal. Payments are processed securely by our authorised payment partner. We do not store card details.
Refunds for cancelled appointments will be processed in accordance with our cancellation policy. Refunds are not available for appointments attended where the service was delivered as booked.
7. Medical Information and Liability
The content on this website is for general informational purposes only and does not constitute medical advice. Always consult a qualified healthcare professional regarding any medical concerns.
Quantum Polyclinic is not liable for any clinical outcomes arising from diagnostic results interpreted by third-party referring clinicians. Our responsibility is limited to the accurate performance and reporting of the diagnostic procedure booked.
Nothing in these terms limits or excludes our liability for death or personal injury caused by our negligence, or for any matter where it would be unlawful to limit liability under Irish law.
